Pre Eclampsia

This document discusses preeclampsia, classifying it as one of four types of hypertensive disorders of pregnancy. Preeclampsia is defined as hypertension and proteinuria developing after 20 weeks of gestation, and can be mild or severe. It complicates 3-8% of pregnancies in Western countries and is a major cause of maternal mortality worldwide, responsible for 10-15% of maternal deaths. Risk factors are discussed, as well as the anatomy of uterine and spiral arteries and the pathophysiology of preeclampsia. Signs and symptoms are also covered. Prevention strategies for high-risk women include low-dose aspirin before 16 weeks and oral calcium supplementation to reduce incidence. Eclampsia

Epidemiology
Pre-eclampsia is a multiorgan disorder that
complicates 3%-8% of pregnancies in Western
countries and constitutes a major source of
morbidity and mortality worldwide.
Overall, 10-15% of maternal deaths are directly
associated with preeclampsia and eclampsia

Preventing Preeclampsia at Women with

Risk Factor
Low Dose Aspirin
Aspirin should be taken in a low dose (75-162
mg/dl),
Who should receive aspirin, in what dose.
Aspirin is more effective in decreasing
preeclampsia:
(i) among high risk women
(ii) when initiated before 16 weeks
(iii) at doses >80 mg/day (75-162 mg/dl)

Mechanism of Aspirin

Oral calcium supplementation (of at least

1g/dl) decreases the incidence of preeclampsia
and gestational hypertension. Maternal death
or serious morbidity was reduced.

ECLAMPSIA
It is extremely difficult to predict which women
with pre-eclampsia will develop eclamptic
seizures. So that the required seizure
prophylaxis in preeclamptic women.

How Does it Work?

Magnesium sulfate mechanism of action in
eclampsia is not well understood. Eclampsia is
thought to occur secondary to ischemic caused
by cerebral vasospasm.[Magnesium sulfate is a
potent vasodilator, particularly in the cerebral
vasculature.In women with pre-eclampsia,
magnesium sulfate has been shown to improve
cerebral arterial circulation.
