I am JEFFREY P. ROSAL, one of the students who graduated last March 2008.
I am from Special Science Class 4A under Ms. Helen Fuentes supervisory.
May I request for Certified True Copies, with school dry seal signed by the principal, of each
of the following documents for College Admission.
High School Diploma Original copy needed
Form 137A and Form 138 Original copy needed
Recognition Certificate (6th Honorable Mention) Original copy needed
Certificate of Good Moral Character
Certificate of Graduation / Letter of Recommendation (from Principal) stating
the name and address of the institution, name of the graduate and date of graduation

Diploma, Form 138 and Recognition Certificate) for reference.
Enclosed in this letter is the SAR-Form 1 with Reference number: 2016-0000-2536 from
Polytechnic University of the Philippines Sta. Mesa for admission requirements.
Your immediate attention on this matter will highly be appreciated.
