AFFIDAVIT OF DISCREPANCY ON DATE OF BIRTH
I, JOYSIE R. PEDROSO, of legal age, married, Filipino and presently residing at
Zarraga Iloilo, after having been duly sworn to in accordance with law, do hereby depose and
say:
1. That OCTOBER 13, 1985 is my date of birth as stated in my registered birth certificate
issued by the local civil registrar of the Republic of the Philippines;
2. That during my childhood years up to the present, I have been accustomed to celebrate by
birthday every 13th day of October;
3. That my real date of birth is OCTOBER 13, 1985 which I use and it depicts in all my
official documents since birth; Attached to this affidavit are copies of my Baptismal
Certificate, Certified True Copy of my Certificate of Live Birth from the Local Civil Registry
of Banate, Marriage Contract and Drivers License marked as Annexes A, B, C and
D, respectively;
4. Only my Certificate of Live Birth issued by the National Statistics Office (NSO) has a
typographical error which reflects October 23, 1985 as my birth date.
5. That I am executing this Affidavit to clear any discrepancy and to correct my real birth date
with the NSO.
IN WITNESS WHEREOF, I have hereunto set my hand this 27th day of April 2016 in
City of Iloilo, Philippines.
