Ineffective Airway Clearance (Retained Secretions)

The patient was having difficulty coughing up secretions but was able to cough up small amounts. Crackles were heard on both lung fields and the patient produced coughs with yellow-green secretions. Nursing interventions included positioning the patient in a semi-Fowler's position, keeping the environment allergen-free, encouraging deep breathing and coughing, increasing fluid intake, and demonstrating breathing techniques. After 8 hours, the patient was able to expectorate secretions readily and understood their condition and treatment plan.
