Edward Hospital & Health Services Post-Offer and Communicable Disease Requirements
Edward is committed to the health and safety of our patients, families, visitors, and employees. The following requirements for our new employees are based on OSHA, IDPH, and JCAHO standards. Prior to start date, all new employees, with the exception of non-KidZone
Edward Health & Fitness Centers staff, must complete and have documented In their Employee Health file the following (see below for details regarding Immunization or disease status): - Urine drug screen - Two-step TB test - Proof of immunity to rubeola (measles), rubella (German measles), mumps, and chicken pox, or initiation of immunization - Completed basic latex questionnaire - Latex allergy test results, if indicated - TB mask fit test, if applicable - Physical exam, if indicated Within one month of start date, all new employees, with the exception of Non-KidZone Edward Health & Fitness Centers staff, must complete and have Documented in their Employee Health file the following: - For those with low or negative rubeola (measles) titer: two Measles/Mumps/Rubella vaccines a month apart - For those with low or negative rubella (German measles) titer: one Measles/Mumps/Rubella vaccine - For those with low or negative mumps titer: two Measles/Mumps/Rubella vaccine a month apart - For those with low/negative chicken pox titer: two Varivax vaccines a month apart New employees with direct patient care must also complete one of the following relative to the Hepatitis B vaccine series: - Acknowledgement and Authorization form signed prior to start date if Hepatitis B vaccine series completed elsewhere. - Acknowledgement and Authorization form signed at Edward Corporate Orientation, to receive the Hepatitis B vaccine series or decline it.
Details Regarding Requirements and Acceptable Documents Rubeola (Measles)
Newly hired Edward employees are required to provide documented proof of immunity to rubeola (measles). This documented proof of immunity must be provided to you by a healthcare provider and must be presented to our Employee Health Department at the time of the initial medical screen. Employees of Edward Health & Fitness Centers do not have to meet this requirement, with the exception of KidZone staff. The documentation of immunity from a healthcare provider must state that you have one of the following: - Rubeola disease - A positive rubeola titer - A series of two rubeola vaccinations or two measles/mumps/rubella vaccinations. If you do not have proof of immunity; our Employee Health Department will draw a blood test (titer) during your initial medical screen: - If your titer is positive, no vaccines are needed. - If your titer is low or negative, you will need two measles/mumps/rubella vaccinations completed one month apart. The first vaccine will be given at the completion of your two-step TB test. The series must be completed within one month of your start date.
Mumps
Newly hired Edward employees are required to provide documented proof of immunity to mumps. This documented proof must be provided to you by a healthcare provider and must be presented to our Employee Health Department at the time of the initial medical screen. Employees of Edward Health & Fitness Centers do not have to meet this requirement, with the exception of KidZone staff. The documentation of immunity from a healthcare provider must state that you have had one of the following: - Mumps disease - A positive mumps titer - A series of two mumps vaccinations or two measles/mumps/rubella vaccinations. If you do not have proof of immunity, our Employee Health Department will draw a blood test (titer) during your initial medical screen: - If your titer is positive, no vaccines are needed.
- If your titer is low or negative, you will need two measles/mumps/rubella vaccinations completed one month apart. The first vaccine will be given at the completion of your two-step TB test. The series must be completed within one month of your start date.
Rubella (German Measles)
Newly hired Edward employees are required to provide documented proof of immunity to rubella (German measles). This documented proof must be provided to you by a healthcare provider and must be presented to our Employee Health Department at the time of the initial medical screen. Employees of Edward Health & Fitness Centers do not have to meet this requirement, with the exception of KidZone staff. The documentation of immunity from a healthcare provider must state that you have had one of the following: - Rubella disease - A positive rubella titer - One rubella or one measles/mumps/rubella vaccine If you do not have proof of immunity, our Employee Health Department will draw a blood test (titer) during your initial medical screen: - If your titer is positive, no vaccine is needed. - If your titer is negative, you will be given a measles/mumps/rubella vaccination at the completion of your two-step TB test.
Chicken Pox
Edward employees are required to provide history of chicken pox disease. Employees of Edward Health & Fitness Centers do not have to meet this requirement, with the exception of KidZone staff. If you do not have history of disease or proof of immunization or titers, our Employee Health Department will draw a blood test (titer) at the time of your initial medical screen. - If your titer is positive, no vaccine is needed. - If your titer is low or negative, you will need two Varivax vaccines completed one month apart. This series must be completed within one month of your start date.
Basic Latex Sensitivity
All new Edward employees must complete a basic latex questionnaire. You will complete this questionnaire at the time of your initial medical screen. - If you answer yes to any of the questions you will need to complete a more extensive questionnaire. You will also be examined by one of our Employee Health physicians to determine if a RAST blood test is needed.
- If Latex allergy testing is done, our Employee Health physician will use the results of the test to determine clearance for work.
Hepatitis B
Employees with direct patient contact must meet this requirement. - If you have already completed the hepatitis B vaccine series you will need to sign a Hepatitis B Vaccine Acknowledgement and Authorization form during your initial medical screen. - If you have begun and not completed, or never begun the series, you will need to sign the Acknowledgement and Authorization form to receive or decline the vaccine. This form must be signed after you have participated in the blood-borne pathogen education/training during Edward Corporate Orientation, or within ten days of your start date, and therefore will not be presented to you at the time of your initial medical screen. - If you consent to receive the Hepatitis B vaccine series, you have 90 days to receive the first vaccine. The schedule for the series will be given to you at the time of your first vaccine. If you are 90 days late in receiving any of the three vaccines in the series, you are considered to have waived the series.
TB TestingNew Employees
Prior to start, all new employees, except non-KidZone Edward Health & Fitness Center staff, must have completed and have documented in the Employee Health file a 2-step TB skin test. A 2-step TB test is two separate TB skin tests given at least one week apart. The 2nd TB test must be read 48-72 hours after administration and must be read by the Employee Health staff. - If you present proof from a healthcare provider of one TB test completed within the past 12 months, one TB test will be given during your initial medical screen. That test must be read within 48-72 hours by Employee Health staff. - If you present proof from a healthcare provider of two negative TB tests within one year with the most recent one within 90 days of employment, no further TB test is needed prior to orientation. - If you have had a previous positive TB reaction, a positive TB questionnaire must be completed. A baseline chest x-ray will also be done if proof of a chest x-ray done in the past year is not provided.
