Custom Surfboard Order Form

DIRECTIONS: Please read carefully


1) Print all of the pages in this order form out from your
printer (1-4). Fill out steps A-F as they are necessary
requirements that are needed to make the board.
2) On pages 3&4, hand draw out the graphics you would
like your board to have (Further directions are given
when you are up to that step)
3) Seal all the pages in an envelope and mail the complete
order to: 201 West Shore Drive, Massapequa, NY,

4) **Once you place order, please email

subject and “Order Confirmation Request” as the
body. Once this email is sent, you will receive a
replied email with the order confirmation and
price as soon as it is received in the mail. **No
board will be made unless this email is sent**

5) Allow 2-3 days until your order is processed and


commenced.
6) If it is not a local pick up, call to arrange a
payment/deliver plan (516)-978-9413

(The rocker refers to the actual bend in the board. The front rocker is
the measurement taken of how much the board bends up vertically,
starting from the center of the board to the front tip. The back rocker is
the measurement taken of how much the board bends up vertically
from the tail. **You can decide not to specify a rocker and leave it to
the shaper if you are unsure.**)
