AFFIDAVIT OF GUARDIANSHIP
I, JOVY N. MORALIZON, of legal age, married, Filipino, and
a resident of Purok Fareview, Riverside, Sudlon, Mati City, after
being duly sworn to in accordance with law, do hereby depose and
state;
1. That I am one and the same JOVY N. MORALIZON, who
caused the execution of this affidavit;
2. That I am the grandmother of Khryzst Mae Moralizon
Somono born on February 5, 2009 who is a minor;
3. That as their grandmother, I stand as their legal guardian;
4. That this affidavit is for the purpose of processing
and/or claiming their Certificate of Live Birth at NSO
Davao City;
5. I am executing this affidavit to attest to the truth of
the foregoing facts for the legal purpose it may serve.
