INVENTORY OF EVIDENCE/PROPERTY SEIZED

THIS IS TO CERTIFY that an inventory was conducted with the following facts of
the operation:

SUSPECT:

1. ROLAND DELA ROSA, 33 years old, married, jobless and a resident of Purok
Talisay, Bonifacio Corregidor, Brgy Zone 3, Digos City.

DATE/TIME OF APPREHENSION: September 05, 2016 at about 8:45 p.m..

PLACE OF APPREHENSION: Purok Tugas, Bonifacio 8th , Brgy Zone 3, Digos City,
Davao del Sur

This is to certify further that the following items were seized during the said operation:

1. One (1) small heat-sealed transparent plastic sachet containing white crystalline
substance suspected to be Metamphetamine Hydrochloride otherwise known as Shabu 7.5
grams with the initials JN and the signature of the poseur-buyer and the initials CP
and the signature of the arresting officers, time, date, place of apprehension and
name/alias of the suspect marked as Exhibit D (subject of the buy bust);

2. One (1) small heat-sealed transparent plastic sachet containing white crystalline
substance suspected to be Metamphetamine Hydrochloride otherwise known as Shabu 7.5
grams with the initials JN and the signature of the poseur-buyer and the initials CP
and the signature of the arresting officers, time, date, place of apprehension and
name/alias of the suspect marked as Exhibit E (subject of the buy bust);

3. Six (6) pieces five hundred peso bill (P500.00) with serial numbers CQ864034,
CS054819, CS080603, GJ968969, GV963698 and GU654615 on which written on it are
the initials JN used as a buy bust marked money, with the initials, signatures of the
arresting officers (PB and AO) with date, time, place and name/alias of the suspect
written in a masking tape (Buy-bust marked money).

4. One (1) piece black wallet containing an Identification Card (Drivers License)
with date, time, place and name/alias of the suspect written in a masking tape.

5. One (1) piece white cellphone, a MyPhone brand with SimPack, with the date,
time, place, name/alias of the suspect written in a masking tape.

This certification is issued pursuant to the requirements of Section 21 (a), Article II


of Republic Act 9165.
