JOINT AFFIDAVIT
OF TWO DISINTERESTED PERSONS

We, RIZAL A. TACLIAD, of legal age, Filipino citizen, and a


resident of Barangay Tuec, Camiling, Tarlac, and SOFINA A. CALI,
of legal age, Filipino citizen, and a resident of Barangay Manaquem,
Camiling, Tarlac, after having been duly sworn to in accordance with
law, do hereby depose and state THAT:

1. We both personally know DELIA BARRACA NOBLE


(Delia, for brevity), who was born on April 1, 1953 to Spouses
Pablo Noble and Anecita Barraca in Pandan, Antique;

2. Delia is also known as MRS. DELIA NOBLE


MARIANO after she got married to Loreto Mariano (Loreto);

3. Delia is a member of the Social Security System of


the Philippines (SSS) until her death on June 2, 2017;

4. When Loreto went to the SSS Camiling Branch to file


for the Death and Funeral Benefit Claim of her late wife, he
was required to submit copies of Delias Birth, Marriage and
Death records;

5. Upon obtaining the required documents, it was


discovered that the recoded First Name of Delia as recorded in
her Certificate of Live Birth is DELLA, instead of DELIA as
appearing in her Marriage Contract and Certificate of Death
including her SSS record. Hence, the discrepancy;

6. We are not related to Delia by affinity or


consanguinity, except that of being her friends;

7. We are executing this Affidavit to attest to the fact


that the names DELLA NOBLE, as recorded in the said
Certificate of Live Birth, DELIA NOBLE, as recorded in the
said Marriage Contract, and DELIA NOBLE MARIANO, as
recorded in the said Certificate of Death, refer to one and the
same person, and that is our friend, Delia;
8. Finally, we are executing this Affidavit to attest to
the truth of the foregoing statements and for all legal intents
and purposes it may serve.

IN WITNESS WHEREOF, we have hereunto affixed our


signatures this 7th day of June, 2017 at Camiling, Tarlac, Philippines.
