TERMS AND CONDITIONS

I HEREBY ACCEPT MY APPOINTMENT SUBJECT TO THE FOLLOWING TERMS AND CONDITIONS:

1. That I affirm to the truth and correctness of all the information contained in my application for employmet which is
made part of this appointment, and any misrepresentation or false information found therein shall sufficiently be a
ground for my immediate dismissal.

2. That I fully understand and agree that since my status of employment is PROJECT EMPLOYEE, that the tenure of
my employment is co-terminus with the completion of the specific item of work of the project where I am assigned
or to the task to which I am assigned, unless otherwise terminated for the other reasons.

3. That I fully understand and agree that should I fail to report continously for seven (7) consecutive days without filing
a leave of absence, it shall mean that I am terminating my project employment contract.

4. That I fully understand and agree that my appointment shall not be construed to mean that I shall have the priority
in the Company’s succeeding appointments for its other projects, nor shall it be construed to mean that I will be a
regular employee after my contract period.

5. That I am assuming full resposibility and accountability of any cash and/or Company property entrusted to me in
line with the performance of my work.

6. That finally, my employment is subject to all the rules, regulations, established practices and policies of L.C.D.C.
which I have acquainted myself and which I am amenable and is subject thereof, and I pledge to strictly uphold,
observe, and abide by the same.

WAIVER

I hereby declare that of my own volition, I applied with Leadway Construction & Development Corporation for employment and that
I am physically fit and able to perform the work I have applied for, that I am not suffering from any illness nor have I suffered from one
which can disqualify or disable me to work or to be employed; that I hereby absolutely absolve Leadway Construction & Development
Corporation from any liability of any nature whatsoever which may arise contrary to my representation herein.
