Caso Clínico - Serums: I. Anamnesis

This clinical case describes a newborn male infant born at 34 weeks gestation via vaginal delivery who presented with difficulty breathing, grunting, and intercostal retractions at 30 minutes of life. On examination, the infant was tachypnic with subcrepitant rales in both lungs and an oxygen saturation of 85% without oxygen. The medical history of the 17-year-old mother included an untreated urinary tract infection during the last 6 days of pregnancy. The clinical findings and risk factors suggest a diagnosis of neonatal respiratory distress, likely due to prematurity and possible maternal chorioamnionitis from the untreated UTI during pregnancy.

Vacunas: Antitetánica 3 dosis. Rotura de membranas: 24 horas previas al parto.
Procesos patológicos durante la gestación: ITU 6 días previos al parto sin tratamiento.

SOMATOMETRIA.
Peso: 2020 g. Talla: 45 cm. Perímetro cefálico: 31 cm.
APRECIACION GENERAL DEL RN.
Mal estado general, taquipneico, con quejido inconstante, tiraje intercostal, tiraje
xifoideo y aleteo nasal (SA: 4 puntos)
EXAMEN REGIONAL.
Piel y Anexos:
Cianosis central. Llenado capilar menor de 3 segundos.
Cabeza:
Cráneo: normocéfalo. Fontanelas: normales.
Cara: Facies no característica, simétrica.
Tórax:
Aparato respiratorio: subcrépitos difusos en ambos campos pulmonares.
Aparato cardiovascular: Ruídos cardíacos rítmicos, regulares, no soplos.
Abdomen:
RHA escasos. Blando y depresible. No visceromegalias. Cordón umbilical: nacarado, dos
arterias, una vena.
Región perianal: ano permeable.
Aparato Genito-Urinario:
De caracteres masculinos.
Examen Neurológico:
Hiporeactivo. Tono disminuido. Llanto: moderado.
