acceptance of this entry for my child hereby waive, release and forever discharge Alangilan
Senior High School administration from all actions, suits, claims and/or demand, criminal, civil
or whatever nature arising out of, or in any way connected from the participation of the
International Conference of Basic Education Research (ICBER) from December 4-6, 2017 at the
Philippine International Convention Center (PICC).

I hereby declare that I have read and fully understood this instrument before signing the
same, and that I have signed the same willingly and voluntarily and with full knowledge of the
legal effects of the same.
