DIS-INTERESTED PERSONS

WE, CARMELITA A. QUIZON, of legal age, widower, Filipino citizen, and


a resident of Purok 3, Barangay Victoria, San Isidro, Isabela and ELENA P.
VALDEZ, of legal age, married, Filipino citizen, and a resident of Barangay
Victoria, San Isidro, Isabela, respectively, after having been duly sworn to in
accordance with law, do hereby depose and state:

1. That we personally know ELENA URBANO ALEJO whose parents


are ALFREO O. ALEJO and MAXIMA URBANO ALEJO;

2. That ELENA URBANO ALEJO was born on 9 September 1970 at


Victoria, San Isidro, Isabela;

3. That it was recently found out that the date of birth of ELENA
URBANO ALEJO was erroneously entered as 11 September 1972 in her Voters
Identification Card issued by COMELEC, San Isidro, Isabela instead of 9
September 1970 which is her true and correct date of birth;

4. That her facts of birth is personally known to us because we are


longtime friends of her parents;

IN WITNESS WHEREOF, WE have hereunto set our hands this 22nd day
of March, 2018 at Santiago City.
