AFFIDAVIT OF LOSS
I, Sgt Sunday C Ruelo 823626 PAF, of legal age, married, Filipino and residing
at Flamingo Village, Upper Calarian, Zamboanga City, Philippines, after having been
sworn in accordance with law hereby depose and say:

That, I am a member in the active service of the Philippine Air Force, Armed
Forces of the Philippines presently assigned at 5322nd/530 ABW, PAF based at Sta
Maria, Zamboanga City.

That, I was awarded 81150 3th Skill Level (PAF FORM 1098) by the
Headquarters Philippine Air Force, Villamor Air Base, Pasay City;

That, said document 81150 3th Skill Level (PAF FORM 1098) was
noticed/discovered missing, believed to be misplaced thus I deemed it to be lost;

That, utmost efforts have been exerted to locate and/or recover the same but all
proved in vain and futile and same is now beyond recovery;

That, I am executing this affidavit attesting to the truth of the foregoing averment
and to inform the proper authorities particularly the Office concerned and to request for
the replacement/reissuance of another in lieu of the lost one.

That, further this Affidavit is executed for whatever legal purpose it may serve.
