Demote dated_____________. Reviewer ______supports the demotion.

In accordance with [Section/Article- Policy or Contract], you are demoted from your [old
job classification] to [new job classification] at [__] % time effective [date] for [reason] as
described in the Notice of Intent to Demote dated ___________. This is an [exempt] (or)
[non-exempt] position. This position is in the [bargaining unit] or [Personnel Policies for
Staff Members]. Your salary is ________________.

A meeting has been scheduled on [date, time, location] with [me], (or) [name of new
Supervisor] to discuss your new job description, performance expectations, work schedule,
and (if applicable) your new [exempt] (or) [non-exempt] status. I will also answer any
questions you may have.

Please review the attached job description and PEM form prior to the meeting.
