AFFIDAVIT OF DISCREPANCY
KNOW ALL MEN BY THESE PRESENTS:

I, RICO ALTAREJOS RAMILO, of legal age, married, Filipino citizen


and a resident of Block 2, Lot 13, Angeles 2 Pasolo, Valenzuela City, after
having been duly sworn to in accordance with law, do hereby depose
and state, that:

1. I am the son of spouse Jaime Ramilo and Alisa Altarejos;

2. I was born on August 15, 1958 at Calipatan, San Jacinto,


Masbate; such facts of birth were duly registered before the
Office of the Local Civil Registrar of San Jacinto, Masbate;

3. In the certified NSO copy of my Certificate of Live Birth, it


appears that my correct date of birth is AUGUST 15, 1958;
however, in my Employment records including my SSS records,
PAGiBIG records and GSIS records, my date of birth was
entered as AUGUST 5, 1959;

4. In view of the foregoing, I sincerely and categorically states


that my correct date of birth is AUGUST 15, 1958 as appearing
in my Certificate of Live Birth and not AUGUST 5, 1959 as
appearing in all of my employment records as well as my
records in the SSS, PAGIBIG and GSIS;

5. I am executing this Affidavit to attest to the truthfulness and


veracity of the foregoing narration of facts; for the purpose of
correcting the entry of date of birth in my employment
records, SSS records, GSIS records and PAGIBIG records;
