AFFIDAVIT OF LOSS

I, KYM ASHLEY A. OLEDAN-VILLARUEL, Filipino, of legal age, married, and with


residence at Poblacion, Buug, Zamboanga Sibugay, Philippines, after being duly sworn to in
accordance with law, hereby depose and state:

1 That I am employed as Attorney III at DENR IX Regional Office, Pagadian City;

2 That I traveled to Manila for the Alternative Dispute Resolution Officers Summit
on March 20-22, 2019. Attached herewith is the Travel Order No. 03-038-19 as
Annex “A”;

3 That on March 19, 2019 I boarded the Cebu Pacific flight no. 5J 772 from
Pagadian City to Manila City at 07:40 AM and for my return flight on March 24,
2019, I boarded the Cebu Pacific flight no. 5J 771 from Manila City to Pagadian
City at 05:15 AM. Attached herewith is the copy of the Cebu Pacific booking
details as Annex “B”;

4 That because I was preoccupied of the activities of the Summit and the consequent
transfers from airport to my residence, I was not mindful of the travel boarding
passes of the said two (2) Cebu Pacific flights;

5 That despite diligent search and efforts to locate my Cebu Pacific boarding passes,
I could not find the same such that I now believe that they are now lost beyond
recovery;

6 As such, I am executing this Affidavit of Loss to attest to the truth of the


foregoing.

IN WITNESS WHEREOF, I have hereunto set my hand this 26th February 2019 at
Pagadian, Zamboanga del Sur, Philippines.
