The name of agency/BDC endorses the following personnel/officials to become the


regular member, and in his/her absence, the alternate member of the Multipartite
Monitoring Team for name of company for CY 2018.

Relative to the recently approved MMT Manual of Operations (MOO), as the official
representatives of the name of agency/BDC, the member must strictly observe the
following:

1. Be able to attend meetings, orientations, trainings, actual monitoring and


reporting activities of the MMT for at least one complete cycle of the Annual
Environmental Protection and Enhancement Program (AEPEP). They are
allowed only two (2) absences per cycle of the AEPEP, inclusive of regular
meetings, orientations, training, actual monitoring and reporting, etc. Alternate
member is entitled to one (1) voting power in behalf of the regular member, in
case of his/her absence because of emergency circumstances.

2. Be able to attend the entire duration of the MMT inspection (2-3 days). In case
of incomplete attendance, the member shall not be authorized to sign official
document nor claim honorarium.

3. Always abide and practice the norm of ethical standards (Code of Ethics) of the
MMT.

This endorsement shall take effect immediately. Given this ____day of September
