Respond thoroughly to each of the following: Number and letter each as it is listed here:

1. Description of activities you engaged in this period.

- Uploading documentation from the Chemical Dependency Treatment (410) office.
- Indexing documentation from the Chemical Dependency Treatment (410) office.
- Traveling to an outside storage that contains HIM documents.
- Good bye party (My last day).

a) Describe your activities in some detail.

Some of the activities that I performed during this period of time include the re-
training on the DCS system in order to make sure that I uploaded and indexed the photos
from the documentation from the Chemical Dependency Treatment (410) office in the
right way. In addition, I went to examine or inspect an outside storage that the the HIM
Department owns with documents that have more than 10 years. The main purpose of this
visit was due to a patient who was asking for their medical documentation, but those
documents have more than 15 years; therefore, we went to that outside storage in order to
get them and also talked about a possible update in the contract. This is because all the
documents that are in that storage are almost 20 years old and; therefore, HIM will have
to destroy them once the documents have 20 years. Finally, I enjoyed my last day with a
good-bye party with food and good memories.

b) Include your reactions to these activities and to your clients and peers.
My reactions were very professional during my visit to the storage and with the
usage of DCS for the indexing, but, internally, I was so happy for all the processes that I
was watching and involved in; it was very exciting to actually see what the real world of
my major was like. This week was more of getting to know the 410 uploading and the
indexing process. In addition, I was very happy during my traveling to the outside storage
of the HIM Office and it was very fun the fact that we saw a sheep in the storage and we
played with the sheep. After we played with the sheep, we started to talk about business
plans with the storage owner in order to let him know that HIM will soon end the contract
because HIM will not need the services anymore for those specific indexed. Regarding
my good-bye party, I was sad and happy, at the same time, for the fact that I finished all
my credit hours, but, at the same time, it was a goodbye to good persons, whom I created
good memories with and learned from so much. But the end is always part of the journey.

c) Were you supervised during these activities?

Yes, I was supervised during these activities. The majority of the time Ms.
Squadrito and Ms. DeFalco were with me, but, if I was in an employee’s office or in my
office, then I was without supervision. In addition, when I went to the outside storage,
then I was with Ms. Squadrito, Ms. Mills, and Ms. Filipovich.

1. If yes, describe the supervision and include your reaction to your supervisor
and supervision.
The supervision was very professional, and, at the same time, I felt their
supervision was great since they always made me feel very welcome. My reaction
to Ms. Squadrito supervision was professional and very friendly and always made
sure I was understanding everything.

2. If no, describe your activities when you were not supervised and your
reactions.
When I was without supervision, I had the same behavior as if I was being
supervised because I wanted to make sure everything I did was done effectively
and responsibly. I had a great respect for Ms. Filipovich and it was never my
intention to lose the trust and faith that she gave to me or that anyone gave to me.

2. What did you expect to learn through these activities?

Related to:

I learned in a lot of depth the reason why this organization is a “non-for-profit”
one and how it really is engaged with employees, doctors, patients, among others in
all aspects. I also learned what “non-for-profit” really means and how Crouse
Hospital is so committed with the community’s needs and resources and with the
citizens that come to obtain healthcare services.
