This document is a Form 60 declaration that must be filled out by any person entering into a transaction specified in Rule 114B of the Indian Income Tax Act of 1962 who does not have a permanent account number. The declaration requires the person to provide their full name and address, details of the transaction including account type and number, the transaction amount, whether they are assessed for tax, and details of the last tax return filed if assessed.
