AFFIDAVIT OF DISCREPANCY

I, _____________________, of legal age, married, Filipino and a resident of


____________________, having been duly sworn in accordance with law, hereby depose
and state:

1. That in my Certificate of Live Birth as recorded


in the Office of the Civil Registrar _____________, it is stated that I was born on
___________________________;

2. That I continuously using ______________as my birthday in my daily


transactions, public records, private documents and other public documents;

3. That when I filled up my registration record for Professional Board Examination


for Teachers on June 1994 as required by the Civil Service Commission, I had
inadvertently and erroneously written the date of my birth as 11 September
1965 instead of 09 November 1965;

4. That I am executing this Affidavit of Discrepancy to attest


to the truth of the foregoing facts and to explain the discrepancy in my
Certificates of Live Birth and data/information appearing in the records of the
Civil Service Office as regards to my date of birth and to facilitate my request for
certification and authentication of my Professional Board Examination for
Teachers Eligibility.
