CIRCULAR

The students and staff who are willing to apply for Passpoft in "Passport Mela" to be held !n

GITAM on 7th September, zOLg should register their names with NSS program officer of their
Institutes and also be in touch with NSS officer for the updates of Passport mela.

The last date for students and staff enrolment is on 03-09-2019. The NSS coordinator,
GITAM is requested to consolidate the list as per Institute wise and send the soft copy to the
Directorate of Student Affairs (Email: [email protected]) on 3'd September, 2019
to make necessary arrangements.

The Passpoft Seva Kendra officer, Visakhapatnam will likely to address students and staff as a part
of orientation on 4th September, 2019. The time and venue will be intimated soon. The Head of
the Institute should circulate to students, staff and NSS program olficer to make the program
successful.

The details of the documents to be submitted and other information are given in Annexure - I.

The students and staff should bring the following documents:

1. Proof of Date of Birth: Photocopy of SSC/CBSE/ICSE Pass certificate duly attested (Seal&
Signature) by the college principal.

2. Custodian certificate to be issued by the college principal on original letter head stating that
the original ceftificates kept in college custody.

3. Day scholars: Original Aadhar or Voter ID or an updated Bank account passbook with the
current residential address.

4. Hostelers: If the student is residing in college hostel and desires to receive a Passport in
college address, a certificate to this effect shall be issued by the college warden/Principal
on the original letter head. Original college ID card, Hostel ID Card is necessary for
hostelers.

5. The students and staff who are willing to apply for Passport in "passport Mela,, to be held
in GITAM on 7th September, TOTS should register their names with NSS program officer of
their Institutes and also be in touch with NSS officer for the updates of passport Mela.

6. The last date for students and staff enrolment is on or before 03-09-2019.

7 By 4th September, 2019 the students are required to download mpas sport Seva app

8. They need to carry a debit card/credit card towards payment of Rs. 1500 for Passport fee at
the time of orientation program to be held on 4th September, 2019.

9. The Institute which is not having NSS program officer then the Head of the Institute
should nominate one of the faculty member who is responsible for the enrollment of their
staff and students details and send the same through soft copy to the Directorate of
student Affairs (Emaill director_studentaffai*6lgitam.ddul on 3.d september,
2019.
