Madam/Sir:

This is in Connection to your Audit Observation Memorandum No. 2019 – 016 dated February 8, 2019
wherein receivable amounting to 940,752.04 have remained uncollected for more than ten (10) years and have
become dormant accounts of the municipality due to the following reasons:

1. No records were available thus the name of accountable officer could not be identified and the
validity of the claim could not be established,
2. Two identified accountable officers were already deceased namely, Gabriel Tabora and Ciriaco
Laroya,
3. No settlement were made by the debtors despite several demand letters sent.

In view of the forgoing, may I request your good office for an authority to write off the dormant
receivable accounts, unliquidated cash advance and fund transfer where settlement/ collectability could
no longer be ascertained. (Please see attached supporting documents).
