TRAINING AGREEMENT AND LIABILITY WAIVER

I, Raymund Benz D. Latonio, a student of the University hereby voluntarily agrees to undergo on-the-job training
which is an academic requirement for graduation, to Bachelor of Science in Civil Engineering under the following
terms and conditions:

a. That I shall abide by all the rules and regulations of _______________________ of Puerto Princesa
City and shall comply with those imposed for on-the-job training, otherwise, I shall be excluded from
further participation;
b. That there is no labor-management relationship between me and the ______________________ of
Puerto Princesa City;
c. That I shall exercise care and diligence in any tasked assigned to me;
d. That I renounce and waive my claim against the ____________________________ of Puerto Princesa
for any injury, that I may sustain or loss that I suffer, personal or pecuniary; in the performance of my
duties or functions while under training; and
e. That I shall be made answerable in any and all liabilities for damage to property or injury to third
persons which may be occasioned by my intentional or negligent acts while in the course of my training.
