LATONIO FERMO, Filipino, of legal age, and a resident of 1206 Rizal Ave.,
Extension, Cogon, Pardo, Cebu City;

3. That I hereby further declare that this transfer , waiver , forfeiture and quitclaim
of rights, interests and participation over the above-described motor vehicle, is
my free and voluntary act and deed ;

4. That I am executing this Affidavit of Transfer, Waiver, Forfeiture and Quitclaim , in


order to formally establish and attest to the foregoing facts, for all legal intents
and purposes ;
