PURPOSE: This Judicial Affidavit of AAAAAAAAAAAA to serve as her
direct testimony, is being offered to prove:
1. That she is the Petitioner in the above entitled case;
2. That she was married to CCCCCCCCCCCC;
3. That she and her spouse obtained a Mutual Divorce on May 21, 2000 in
Japan;
4. To identify documents relevant to the said Petition.

QUESTIONS were propounded by Petitioner’s counsel Atty.

translated in English.

Barangay ________ stating that I am a bonafide resident of said barangay.

Shi, Saitama Ken.

MARRIAGE?
ANSWER: Yes, I have with me a copy of our Report of Marriage.

CERTIFICATE OF ACCEPTANCE OF DIVORCE

ANSWER: I want that the divorce we obtain in Japan be judicially
recognized here in the Philippines, Atty.

ANSWERS: No more, Atty. Me and my estranged Japanese husband
have no communication since we obtained our divorce on the year 2000
