JUDICIAL AFFIDAVIT
PURPOSE: This Judicial Affidavit of AAAAAAAAAAAA to serve as her
direct testimony, is being offered to prove:
1. That she is the Petitioner in the above entitled case;
2. That she was married to CCCCCCCCCCCC;
3. That she and her spouse obtained a Mutual Divorce on May 21, 2000 in
Japan;
4. To identify documents relevant to the said Petition.
QUESTIONS were propounded by Petitioner’s counsel Atty.

fully understands, while ANSWERS were given/made by witness in Tagalog and
translated in English.

Barangay ________ stating that I am a bonafide resident of said barangay.

3. QUESTION: WHO IS THE PETITIONER IN THIS CASE?
ANSWER: I am the petitioner, Atty.

ANSWER: I was married to CCCCCCCCC of legal age, and resident
of Saitama Province, Japan, Atty.
6. QUESTION: WHEN AND WHERE WERE YOU MARRIED?
ANSWER: Our marriage was solemnized on October 23, 1995 in Hanyu
Shi, Saitama Ken.
7. QUESTION: DO YOU HAVE ANY PROOF OF SAID
MARRIAGE?
ANSWER: Yes, I have with me a copy of our Report of Marriage.
8. QUESTION: I AM SHOWING YOU A REPORT OF
MARRIAGE, HOW IS THIS RELATED TO THE DOCUMENT
THAT YOU MENTIONED?

9. QUESTION: WHAT IS THE YOUR STATUS RIGHT NOW?
ANSWER: We are already divorced and living separately now, Atty.
10.QUESTION: WHO FILED FOR DIVORCE?
ANSWER: Both of us, Atty. We obtained a Mutual Divorce on May 21,
2000.
11.QUESTION: DO YOU HAVE ANY PROOF THAT BOTH OF
YOU WERE INDEED DIVORCED?
ANSWER: Yes, Atty.
12.QUESTION: I AM SHOWING YOU AN AUTHENTICATED
CERTIFICATE OF ACCEPTANCE OF DIVORCE
(JURISHOMEISHO), IS THIS THE SAME DOCUMENT THAT YOU
ARE REFERRING TO?

OFFICE OF THE CITY CIVIL REGISTRAR OF MANILA?

MANIFESTATION: Your Honor, the Certification issued by Office
of the City Civil Registrar of Manila is hereby being offered and previously
marked as our EXHIBIT “____”.
15.QUESTION: WHAT IS THE RELIEF YOU SOUGHT TO ASK
BEFORE THIS HONORABLE COURT?
ANSWER: I want that the divorce we obtain in Japan be judicially
recognized here in the Philippines, Atty.
16.QUESTION: DO YOU STILL HAVE ANY COMMUNICATION
WITH YOUR ESTRANGED HUSBAND?
ANSWERS: No more, Atty. Me and my estranged Japanese husband
have no communication since we obtained our divorce on the year 2000

ESTRANGED HUSBAND?
ANSWERS: Our marriage was not blessed with any issue.
18.QUESTION: DID YOU ACQUIRE ANY PROPERTY, REAL OR

ESTRANGED HUSBAND?
ANSWER: None, Atty.
19.QUESTION: WHY DO YOU WANT YOUR DIVORCE TO BE
JUDICIALLY RECOGNIZED BY THIS HONORABLE COURT?
ANSWER: I want our divorce to be judicially recognized here in the
Philippines because I intend to remarry, Atty.
