AFFIDAVIT OF DISCREPANCY IN NAME

I, RAMON KHALIL C. ERUM IV, of legal age, Filipino with residence in no. 99
Isla Suerte, Bucana Davao City, under oath state:

1. That my name is RAMON KHALIL C. ERUM IV as stated and registered


in my birth certificate issued by the office of the Civil Registry of the
Municipal registry of Carmen, Davao Oriental.
2. That my personal records, transactions and communications are all
under the name of RAMON KHALIL C. ERUM IV and not RAMON KALIL
C. ERUM IV;
3. That the name RAMON KHALIL C. ERUM IV and RAMON KALIL C.
ERUM IV refer to one and the same person, the here in affiant;
4. That I am executing this affidavit to attest the truth of the foregoing
facts and use the same for whatever legal purpose it may serve
