AFFIDAVIT OF NON-EMPLOYMENT

I, CHERUB DIGAMON DARAMAN, of legal age, single,


and a resident of Brgy. Dagocdoc, Tandag City, Surigao del Sur,
after having been duly sworn to in accordance with law hereby
depose and say:

1. That I have applied for the correction of my records


specifically my gender from Female to Male in my
Certificate of Live Birth;

2. That as one of the requirements for the said processing, I


am required to divulge my employment status;

3. That as of the present time, I am not employed in any


capacity at any private or government corporation,
company, and to any individual person;

4. That I am only a College Student at University of San


Carlos, Cebu City;

5. That I am executing this Affidavit of Non-Employment to


attest to the truth of the foregoing and to support my
Application for Correction of my records on my
Certificate of Live Birth.

IN WITNESS WHEREOF, I have hereunto set my hand this
