We invite you to participate in a research study entitled “VEGETABLE AS THE MAIN
INGRIDIENT OF LUMPIA WRAPPER.” The purpose of this research study is to
know if Lumpia Wrapper will be marketable when using vegetable as its main
ingredient.
We will keep the information you will provide confidentially; however, University of
Perpetual Help System Dalta and our research teacher may inspect and copy records
pertaining to this research. To help protect your confidentiality, we will use a false
name and not your real name to identify your study information. Your study name will
not be linked to your real name. All information will be destroyed at the end of the
study. No identifiable quotations will be used in any papers or reports of this study. If
we write a report about this study, we will do so in such a way that you cannot be
identified.
You may be uncomfortable answering our questionnaire. You may skip any questions
you do not wish to answer and you may end the interview at any time. You will not
benefit personally from being in this study. However, we hope that others may benefit
in the future from what we learn as a result of this study.
You will not have any costs for being in this research study. You will not be paid for
being in this research study. Taking part in this research study is completely voluntary. If you
decide not to be in this study, or if you stop participating at any time, you won’t be penalized
or lose any benefits for which you otherwise qualify.
