I would like to request for cash advance in the amount of One hundred one thousand three
hundred thirty three pesos and 33 centavos (Php 101,333.33) corresponding to the First Release of
MOOE Fund of Agtughangin Integrated School (Elementary), District of Madalag intended for payment
of the following expenditures reflected in the approved Annual Implementation Plan for CY 2020.

1. Expenses for Graduation/Moving Up 2,900.00
2. Improvement of School Gate and Repainting of Signage 25,600.00
3. Repair/Repainting of School Building 12,000.00
4. Provision of Material Recovery Facility 22,000.00
5. Travel of SH/Teachers/Pupils 12,000.00
6. Electricity Expenses 3,533.33
7. Telephone Expenses (Mobile) 1,200.00
8. Supplies related to Covid-19 20,000.00
9. Installation of water line from water tank to school gate 2,100.00
( for handwashing)
Total Amount Php 101,333.33
