Boiler Turbine Generator: Unit Trip Report-Draft STPP, 2X600 MW

The unit tripped on August 3rd, 2018 at 16:49 hrs due to a loss of high pressure condensate feedwater pump B which caused turbine trip oil pressure to drop, triggering the turbine and generator trips. Analysis found the pump tripped due to a flashover in its motor terminal box. Improvements are recommended to address pressure drops on pump changes, pump trip logic, superheater spray block valves, drum pressure rise issues after trips, and deviations in HP bypass valve openings. Rectification work is assigned to various departments with target completion dates.

5.0 Reasons for trip /outage: (first up)

At 16:49:30:400 hrs Turbine tripped on “TP1 Reverse power ½ Relay Protection” due to trip oil
pressure very low (< 4+0.5kg/cm2) as HPCF pump-B tripped at 16:49:27 hrs due flash over occurred in
the motor terminal box.

5.1 Similar Occurrences in the Current & Previous Financial year: Nil

5.2 Occurrence/ Action leading to trip

At 16:49:27hrs Unit was running normal at 604MW Load with Mill–B, D, E, F,G & H were in service,
coal flow 320TPH. Suddenly HPCF-B tripped and HPCF-1 did not came into service immediately due to

which turbine trip oil pressure dropped to 0.38 kg/cm2 from 8.14 kg/cm2 as result Turbine tripped on
“Reverse power ½ relay Protection” at 16:49:30:400hrs and Generator tripped on “Turbine trip to
generator trip protection” at 16:49:34hrs. At 16:49:49hrs boiler tripped on “drum level very low
protection” (<-375mm).

6.0 Analysis of trip:

At 16:49:27hrs Unit was running normal at 604MW Load with Mill–B, D, E, F,G & H were in service,
coal flow 320TPH. Suddenly HPCF-B tripped (due to flash over in the motor terminal box) and HPCF-1
came into service after 4 seconds as Start command for HPCF-1A released from DCS on HPCF
pressure low only, not on tripping of (Running pump) HPCF puimp-1B as its off feedback not came in
DCS(not recorded in SOE). HPCF accumulator’s bladders are in damaged condition. Moreover, Pump-
1A developed pressure after 1.2minutes of starting which resulted in low control fluid pressure and
finally turbine trip oil pressure dropped down to 0.38kg/cm2 from 8.14 kg/cm2 as result Turbine
tripped on “Reverse power ½ relay Protection” at 16:49:30:400hrs and Generator tripped on
“Turbine trip to generator trip protection” at 16:49:34hrs. At 16:49:49hrs boiler tripped on “drum
level very low protection” (<-375mm).

7.0 Observations
1. HPCF pump-1B tripped due to flash over in motor terminal box.
2. Pressure drops during changeover of HPCF pump.
3. HPCF pump-1A ON feedback came 4 seconds later after tripping of HPCF pump 1B.
4. Super heater spray block valve not closed even after MFT.
5. Drum pressure shoot up after unit tripping for the following reason.
(1)HP Bypass valves opened but closed due to Condenser pressure high.
(2)ERV was not in service
(3) Fast open of HP bypass not acted
6. MS LHS safety valve MSV-7 popped up and exhaust silencer pipe broken after flange welding.
7. BCW-B (S/b) Pump auto start irrespective of no pump tripping & No DP low.

8.0 Oil consumption:

Before synchronization oil consumption (LDO/HFO) = 10.99 KL /15.27 KL
After synchronization oil consumption (LDO/HFO) = 0 KL / 7.63KL
Total oil consumption (LDO/HFO) = 10.99 KL/22.91 KL
