PES Statement Exercise Practice Activity 1
Instructions:
Create one simple PES statement containing 20 words or less for each of the following
scenarios.
Case 1
A 28 year old Mexican American male who works as a financial planner was diagnosed
with diabetes 5 years ago. Since that time his weight has increased to 210#. He is 5’6”
tall. He has hypertriglyceridemia (324 mg/dL) and his fasting blood sugar is usually
about 180 mg/dL. He likes all foods, eats three large meals and snacks throughout the
day on chips and soda.
Case 2
An 87 year old white female was admitted to hospice with renal cell cancer. Her intake
is poor. She will take some sips of liquids but does not take solid foods. She has lost a
great deal of weight, and looks emaciated. She and her family want comfort measures
only.
Case 3
A 37 year old housewife receives dialysis three times weekly. Her diet order is three
grams of potassium and three grams of sodium daily with a fluid restriction of 750 ml
daily. Her monthly labs come back and her BUN and Cr are as expected (BUN 63 mg/dl,
Cr 6.1 mg/dl) Her K+ is up to 6.1 mg/dl from 5.7 mg/dl last month. Because she has
been cautioned not to gain weight, she has been snacking on fresh fruit from her
mother’s garden.
