you give your informed consent. By signing this informed consent statement you are
indicating that you understand the nature of the study and your role in that study and that
you agree to participate in the research. Please consider the following points before
signing:
 I understand that I will be provided with an explanation of the research in which I
participated and be given the name and telephone number of the individual/s to
contact if I have questions about research.
 I can choose whether or not my identity be linked with my data.
 I understand that a transcript of my data will be produced
 I understand that the transcript of the data will be analysed by the researchers.
 I also understand that my word may be quoted directly.
 I agree that the researchers may publish documents that contain quotations by me.
 I understand that participation in this study is not required, is voluntary, and that
after any individual research project has begun, I may refuse to participate further
without penalty.
