AFFIDAVIT OF DISCREPANCY (DATE OF MARRIAGE OF


PARENTS)

I, DELA CRUZ, of legal age, Filipino, single and presently residing at


Rimando Road, Baguio City after having been duly sworn to in accordance
with law, depose and state:

1. That I was born to the spouses and on September 15, 1993 in


Kabugao, Apayao, Philippines, as evidenced by the Certificate of Live
Birth issued by the Civil Registrar of Kabugao, Apayao;
2. That in my birth certificate, the date of marriage of my parents was
erroneously written as August 19, 2012, instead of the true and
correct date August 20, 2012 as evidenced by the Certificate of
Marriage of my parents issued by the Civil Registrar of Baguio City,
hereto attached as Annex “A”;
3. That the discrepancy was a mistake done inadvertently; and,
4. That I am executing this affidavit to attest to the truth of the foregoing
facts.

IN WITNESS WHEREOF, I have hereunto set my hand this 17th


day of January 2017 in Baguio City, Philippines.
