Subject: AUTHORIZATION LETTER

Dear Ma’am/Sir:

I, James Cherish D. Gumela, is sending you this letter as notification that I am


allowing/authorizing the bearer of this letter, Ms. Richel P. Albite, my friend and my co-teacher
for the renewal of my PRC ID in your office. Ms. Albite will also claim the said PRC ID in your
office as soon as it is available for claiming on my behalf.

I won’t be able to process the renewal and the claim thereof personally because I am in United
States of America right now working as a math teacher. Hence, I asked her to bring this letter
together with my identification card as valid proof. I am also giving her the right to sign any
papers that have a connection with the renewal and claiming of my PRC ID.
