AFFIDAVIT OF DISCREPANCY

I, xxx, of legal age, married, and a resident of Davao City, Philippines, after having been
duly sworn to in accordance with law, depose and state that:

“xxx” to “xxx” as evidenced by the certified true copy of my Certificate of Live Birth which
is attached hereto as Annex “A”;
