AFFIDAVIT OF LEGITIMATION
We, Juan C. Dela Cruz and Maria C. Mercedes, both of legal age,
presently residing at Poblacion II, Gerona, Tarlac, and we after having
been duly sworn to in accordance with law, do hereby depose and say:

1. That we are the parents of Mikaela M. Dela Cruz, who was born
on April 18,2018 in Tarlac Medical Center, Tarlac City, as
evidenced by the attached copy of the child’s Birth Certificate,
marked as Annex “I”;

2. That our daughter, Mikaela M. Dela Cruz, was born out of wedlock;

3. That at the time of the conception of our child, there was no legal
impediment for us to contract marriage;

4. That the birth of our daughter was registered/reported at the


Office of the Local Civil Registrar at Tarlac City on April 24, 2018;

5. That we were married on February 12, 2021 in Tarlac City, Tarlac,


as evidence by the Marriage Certificate, marked as Annex “II”;

6. That by virtue of my subsequent marriage with Maria C.


Mercedes, the said child is now legitimated by operation of law;
and

7. That we are executing this affidavit attest to the foregoing facts,


for purposes of complying with the requirements in applying for
the legitimation by subsequent marriage of our daughter, Mikaela
M. Dela Cruz, and for all legal intents and purposes.

IN WITNESS WHEREOF, we have hereunto affixed our signatures


below, this 21st day of February, 2021 at Tarlac City, Tarlac.
