I, FRANCIS RAE G. CAMTUGAN II, of legal age, single, Filipino citizen, and a resident of
203 Cedar Peak, Mabini corner General Luna, Barangay Kabayanihan, Baguio City, Philippines,
after being duly sworn to in accordance with law, depose and state that:

1. I am a holder of a passport I.D. P0382450A. In said Identification Document, my name


appears as FRANCIS RAE GAYGAY-EO CAMTUGAN II;

2. In my certificate of LIVE BIRTH, my name appears as FRANCIS RAE GAYGAY-ED


CAMTUGAN II which is the correct entry;

3. That the name in my PASSPORT should appear as FRANCIS RAE GAYGAY-ED


CAMTUGAN II and not FRANCIS RAE GAYGAY-EO CAMTUGAN II;

4. I am executing this affidavit of discrepancy for all legal intents and purposes it may
serve.
