MEASUREMENT OF VITAL SIGNS


Weight: 75 kg Height: 5’9” Pulse: 95 bpm
Temp: 36.7 C Resp: 18 bpm BP: 120/80 mmHg

Patient A.B is a 76 years old with a history of high blood pressure and the patient has
allergy to seafoods. Significant childhood diseases or a history of immunizations were not
remembered. Hospitalization had been only when he stroked. His Sleep became disturbed. Pills
for sleeping comfortably had been prescribed due to disturbed sleeping pattern. He had ill-fitting
dentures as he had lost weight recently. Cognitive impairment was present. Nutrition on his diet
included more vegetables and fish. His dentures did not fit well due to his loss of weight and so
he would rather have a diet easily swallowed. Patient A.B had been having an increasing
memory loss or memory gap and he can move around for his daily functions such as walking
ansd exercising. His memory too became poorer. Executive functions slowly dwindled and he
was unable to do things that is heavy. However, He used to do things because according to him it
helps him to become productive. Moreover, he seemed to withdraw from his friends and family
whom he used to meet frequently and spend his time laughing and conversing within better days.
Recently he had stroke due to alcoholic drinks. This had upset him further though that phase had
been successfully passed over. Even with multiple complaints and problems he had been
reluctant to approach anyone for help. He attributed his complaints to aging. It was at the behest
of his son that he had now agreed to come. With the son’s assistance, the problems were all
hopefully revealed. Past Medical history it said that he had been taking anti-hypertensives for a
year now.
