PANDEMIC UNEMPLOYMENT ASSISTANCE MONETARY REDETERMINATION

NOTICE OF ADDITIONAL BENEFIT ENTITLEMENT

Attention THOMAS E MYERS,

Unemployment Insurance Agency records show that you filed a claim for Pandemic Unemployment Assistance
(PUA) and your claim was allowed. A redetermination may have been issued later notifying you of a change in
the number of weeks that you are entitled to receive benefits on that claim.

State and federal law requires that any time there is a change in your benefit entitlement, a monetary
redetermination must be issued to notify you of the change. This monetary redetermination is being issued to
notify you of your new benefit entitlement on your PUA claim.

Effective March 11, 2021, the American Rescue Plan Act (ARPA) was signed into law. The ARPA provides for an
additional 29 weeks of PUA benefits making PUA payable for up to a maximum of 79 weeks. The additional
weeks of PUA and supplemental FPUC payments are payable through the week ending September 4, 2021.

If you have already been paid benefits on a regular unemployment claim, Extended Benefits (EB), or you have
already been paid some weeks of PUA, those weeks will be deducted from your maximum PUA entitlement. Log
into your Michigan Web Account Manager (MiWAM) to see the exact number of weeks of entitlement that you
have remaining.

The ARPA also provides for an extension of Federal Pandemic Unemployment Compensation (FPUC). FPUC is
a supplemental $300 benefit paid in addition to your weekly benefit amount. FPUC is payable on all claim types if
the amount of the payment on your underlying claim is at least $1 for the week. The additional weeks of FPUC
are also payable through the week ending September 4, 2021.

This redetermination is effective beginning March 22, 2020.

Claim Information
• Benefit Year Beginning (BYB): March 22, 2020
• Your weekly benefit amount: 160.00
• Your maximum weeks of entitlement are 79.00. Log in to your MiWAM account to review your remaining
balance.
Your weekly benefit amount (WBA) was calculated based on the income amount you provided. If you did not
provide any income, you will receive the minimum PUA weekly benefit amount of 160.00. If you stated in your
application that you had previously had income but did not provide proof of that income, you are required to
submit proof. If the proof is not provided, or is not sufficient to establish your net income, your weekly benefit
amount will be reduced to the minimum of 160.00 per week. UIA will send you an income verification notice.

Friend of the Court

PUA and FPUC benefits are subject to Friend of the Court child support deduction orders.

New PUA Claim Requirements

PUA claimants are required to submit proof of employment or self-employment that is the basis of the PUA claim
filing. UIA will send a separate notice requesting the documentation. The request will include additional
information, the deadline date for submission, and examples of acceptable proof. For claims filed or reopened
prior to January 31, 2021, you will have 90 days from the date of the notice to submit your proof of employment.
For new, original PUA claims filed on or after January 31, 2021, you will have 21 days from the date of the
request to submit your documentation.

If you do not submit the proof or if the proof is not sufficient to establish that you were employed or self-employed,
your PUA benefits will stop and you may have to repay PUA benefits that you already received.

In addition to proof of employment, you will be required to verify your identity if you have not already been
required to do so. UIA will send a separate request for your identity verification. The notice will provide additional
information and instructions on how to submit your verification, the deadline for submission, and examples of
acceptable proof. You have 10 days from the date of the notice to submit the requested documents to establish
your identity.

If you do not respond or the documents you provide are not sufficient to verify your identity, your benefits will be
stopped, and you may be required to repay benefits already received.

Under provisions of the Michigan Employment Security (MES) Act, benefits determined payable will be paid even
