I/We hereby willingly and voluntary give consent the participation of my/our
son/daughter Gracelyn Adino I am ABM strand students from the Grade: 12 Section: Lavender
for his/her participation foe their 10 days Business Enterprise Stimulation.
I have considered the benefits that my son/daughter will derive from his/her
participationin this activity provided that due care and precaution will be observed to ensure
