AFFIVDAVIT OF CONSENT

We, CATLINO BALTAZAR, SIMON BARROGA, and ALFREDO BALTAZAR, all of legal age, Filipino
Citizens and residents of Brgy 10, San Miguel, Sarrat, Ilocos Norte, Philippines after having been duly
sworn to in accordance with law do hereby voluntary depose and state that:

1. a. I, BELARMINO BALTAZAR, the heir of the late CATALINO BALTAZAR, the lawful owner of a
parcel of lands known as Lot 18422 and Lot 18424, of the Sarrat Cadastre, situated at Brgy. 10 San
Miguel, Sarrat, Ilocos Norte, containing an area of One Thousand Seven Hundred Sixty Nine
(1,769) square meters, more or less declared under Tax Declaration NO. 20-0018-01002(Lot
18422) and One Thousand Five Hundred Sixty Nine (1569) square meters, more or less declared
under Tax Declaration No. 20-0018-01004 (Lot 18424).

b. I, GREGORIA BARROGA, married to the late SIMON BARROGA, the lawful owner of a
parcel of lands known as Lot 18423, of the Sarrat Cadastre, situated at Brgy. 10 San Miguel, Sarrat,
Ilocos Norte, containing an area of One Thousand Five Hundred Ninety Five (1,595) square
meters, more or less declared under Tax Declaration NO. 20-0018-01003.

c. We, BEJAMIN BALTAZAR, JEFFREY BALTAZAR, PAULINO BALTAZAR, the heirs of the late
ALFREDO BALTAZAR, et al, the lawful owner of a parcel of lands known as Lot 18425, of the Sarrat
Cadastre, situated at Brgy. 10 San Miguel, Sarrat, Ilocos Norte, containing an area of One
Thousand Five Three hundred Sixty Six (1,595) square meters, more or less declared under Tax
Declaration NO. 20-0018-01005.

2. We hereby give our consent to SPS. MAGNO & ELENA LAGAN, Likewise of legal age, married
Filipino citizen, and a resident of Brgy. 10 San Miguel, Sarrat, Ilocos Norte, to construct a residence
house within a portion of the aforesaid lots.

3. We are executing this affidavit to attest to the truth of all foregoing facts and to request the
concern authorities/agencies to consider the same, and for any legal purposes.
