In accordance with the DepEd’s mandate to protect and promote the right to and access to quality basic
education, DepEd collects various data and information, including personal information from previous subjects using
different systems, online or offline.

In this regard, the undersigned parent VOLUNTARILY: (a) consent that the school will proceed the various data
and information, including personal information of my child, (b) allows posting of data and information, including
personal information of my child in the official FB page of the school and the FB groups that will be formed; (c) agrees
that pictures and/or videos where my child is ncluded will be utilized by the school for educational purposes and allows
that it will be posted in the official social media accounts of the school.
