MEMORANDUM OF AGREEMENT
KNOWN ALL MEN BY THESE PRESENT:
This memorandum of agreement entered by and between:
TABANGOHAY ELEMENTARY SCHOOL with the office address in Brgy.
Tabangohay Alangalang, Leyte represented by the School Head, OSCAR D. MATELA, JR.
hereinafter referred to as the “FIRST PARTY”.
-and-
MR. EDWIN B. DIAZ, with residential address in Brgy. Tabangohay, Alangalang,
Leyte, of legal age, single, and Filipino hereafter referred to as the “SECOND PARTY”.
WITNESSETH, that:
WHEREAS, The SECOND PARTY has volunteered and selected by the school for the
“PROJECT BISIG-KLETA” for distribution and retrieval of self-learning
modules to the learners who are living far from Tabangohay Elementary School.
WHEREAS, The FIRST PARTY has a knowledge that the contract he signed hereinto is
voluntary and has no monetary compensation form the school.
NOW THEREFORE, for and in consideration, the parties are hereby agreed as follows:
The FIRST PARTY shall:
Ensure that the self-learning modules are complete and ready to be distributed to the
recipient learners.
Ensure the safety of the volunteer.
Provide bicycle to the SECOND PARTY.
The SECOND PARTY shall:
Shall receive a bicycle to be used in the distribution and retrieval of self-learning
modules.
Distribute and retrieve self-learning to the identified learners as scheduled.
IN WITNESS HEREOF, the parties hereinto affix their signature this _________ day of
