Activity 3.

Developing a Family Nursing Care Plan

Introduction: A FNCP is the blueprint of the care that the nurse designs to systematically
minimize or eliminate the identified health and family nursing problems through explicitly
formulated outcomes of care (goals and objectives) and deliberately chosen set of
interventions, resources and evaluation criteria, standards, methods and tools. Specific features
of NCP are based on the concept of planning as a process. The following are characteristics of
NCP:

1. It focuses on actions which are designed to solve or minimize existing problem.


2. It is a product of a deliberate systematic process
3. It relates to the future. It utilizes events in the past and what is happening in the
present.
4. It is based upon identified health and nursing problems
5. It is a means to an end, not an end in itself.
6. Nursing care planning is a continuous process, not a one-shot deal. The results of
evaluation of the plan’s effectiveness trigger another cycle of planning.

Qualities of a NCP:

1. It should be based on clear, explicit definition of the problem/s.


2. A good plan is realistic
3. It is prepared jointly with the family
4. It is most useful in written form. It is a means of communication among nurses and
between nurses and other members of the health team.

Steps in developing a FNCP

Step 1: Prioritize the health conditions and problems based on the nature of condition or
problem; modifiability, preventive potential and salience.

Step 2: Define the goals and objectives of care. Formulate the expected outcomes to be
observed to show problem is prevented, controlled, resolved or eliminated; client’s response/s
or behavior. Be sure to state specific, measurable client-centered statements/competencies.

Step 3: Develop the intervention plan. Decide on:

a. Measures to help eliminate barriers to performance of health tasks and underlying


cause/s of non-performance of health tasks
b. Family-centered alternatives to recognize/detect, monitor, control or manage health
conditions or problems.
c. Determine methods of nurse-family contact
d. Specific resources needed

Step 4: Develop the evaluation plan. Specify criteria/outcome based on objectives of care and
methods or tools used.

Analyze your answers in Activity 1 and Activity 2 and fill in the tables below.
