REMINDERS to all designated/authorized personnel

A. RECORDS Matters

All documents are being received and released at the Records Section, and to facilitate easy encoding
and quick retrieval of the needed documents, this Office requests that all documents being submitted
should have

a. at least two (2) copies of transmittal letter indicating the name of the proposed recipient (office
unit or name of the official) for fast and easy segregation of incoming documents and to avoid
missending to the wrong office/official/employee

b. name of school /district of the researcher on RESEARCH PROPOSALS, etc

1. DIPLOMA RECONSTRUCTION

Submit request stating the reason/s for the reconstruction.

Kindly coordinate with Dan Harvey Castro for the revised format

2. CAV (Certification, Authentication and Verification) of Academic School Records (please


refer to DepEd Order No. 48, 2017)

In as much as the indorsement is already addressed to the Regional Director, the request should
be directly submitted to the Regional Office, thus, there is no need to be indorsed by the SDS.

Requirements:

1. Indorsement letter duly signed by the School Principal/School Head


2. Diploma if graduated
3. Certificate of completion/graduation
4. Duly signed Form 137
5. 2 pcs passport size picture

3. CORRECTION OF NAME (please refer to DepEd Order No. 48, s. 2017)

Requirements:

1. Indorsement duly signed by the School Head/School Principal


2. Birth certificate issued by PSA
3. Form 137
4. Affidavit of discrepancy
5. Affidavit of two (2) disinterested persons

CASH (Cashier)

Please always submit CD for payment of incentives, benefits, etc. that requires the names and
other information of all teaching and non-teaching personnel in your respective schools/districts for
easy checking and quick processing.

Remove the names of employees who were already separated from the service whose bank
account automatically close, to avoid floating of the amount intended to be credited to their closed/old
account. The new account may be used if already available.
Pag-IBIG (Pagtutulungan sa Kinabukasan: Ikaw, Bangko, Industriya at Gobyerno

Employees who incurred more than 30 days leave without pay should coordinate immediately to Pag-
IBIG office regarding payment that should be made by the employer and employee, to avoid gap in the
remittances which has a very big impact on provident claims benefit. When there is a gap, the
computation will start again and not from the first day of service as the employee expects.
