Dear Sir/Madam:

This letter is to request for verification of Employer SS account since it is mandatory for
employers to use the system-generated PRN to facilitate the immediate and correct posting of
remittances and loan payments. As of this moment, our remittances are put on hold due to
unavailability of the account. We have tried to enroll an account online, but we cannot match the other
information needed by the system. We humbly ask for your consideration to verify or reset when there
is existing account under our employer number and assist us in enrolling an account and could start
generating the payment reference number.
