Demand Letter HOSPITAL BILL-PAO-Castillo

Re: Demand Letter to Immediately


Release Patient

Dear Sirs/Mesdames:

This is in connection with the request for legal assistance of Ms. SIONY B.
CASTILLO of Barangay City Heights General Santos City, on behalf of her
common law husband, ARNOLD MIGUEL ORTIZ. According to Ms. Castillo, her
common-law husband was already issued a discharge clearance by his doctor.
However, your hospital is still withholding his discharge for failure to settle his
hospital bills.

Mr. Ortiz and his family are doing everything to fully settle their accounts
but could not produce the full amount at this time. However, their immediate
concern is for Mr. Ortiz to be allowed to leave the hospital so that his bills should
not pile up. They are willing to execute a promissory note to cover their
obligation. (Note: as to date the outstanding balance of the patient is P11,224.05)

In view thereof, we hereby formally request/demand that your hospital


Administrator or duly authorized representative release the said patient upon
receipt of this letter.

Should you fail to do so, Mr. Ortiz and his family shall be constrained to
file the necessary and appropriate criminal, civil and administrative actions
against you in accordance with the New Civil Code of the Philippines and
Republic Act No. 9439, otherwise known as "An Act Prohibiting the Detention of
Patients in Hospitals and Medical Clinics on Grounds of Nonpayment of Hospital
Bills or Medical Expenses". Likewise this is in violation Ortiz Constitutional rights
of non-imprisonment due to debt.

Attached herewith is the promissory note executed in favor of Mr. Arnold
