1. We, M/s OFFSHORE MARINETECH PVT. LTD. are pleased to inform that (02) nos of erew personnel arerequiredto be sentto
Offshore Installation SHP Process Complex of MH Asset for the job of Replacement of Utility System at SH Complex against the NOA
No. MR/MH/MM/LSTK/Piping Utility Sys/289711/18/Y15SC18006/90300099525 Dtd. 15.11.2021 awarded to us by ONGC.

2. We have read the Standard 0Operating Procedure's prepared by ONGC for sending the contractual crew members to offshore
Installation / Rig / Plant / Vessel.

3. We hereby, confirm that allinstructions and conditions mentioned in Standard Operating Procedure issued byONGC regarding
Contractual crew changes are acceptable to us. We shall strictly follow the social distancing protocol, use of PPE as well as the
guidelines provided by ONGC in this regard.

We have noted that strict action wil be taken for any violations/deviations on the above and will be subject to ONGC Management's
discretion. Our full compliance and cooperation shall be provided to ONGC to carry out Contractual crew change as per the
&
guidelines issued by ONGC. Any violations by contractors and/or Service Providers will face consequences and contracts terms
conditions will be applied subject to a maximum level to Suspension and /or Cancellation of the Contract/Agreement

5. We have noted the following points regarding crew change procedure and confirm to hereby abide by them. to the fullest
a.
Crew personnel under quarantine shall not be allowed to leave the hotel room and its premises without the consent of ONGC
once Check in by hotel has been completed.

b. In case, Crew member is required to check out of the hotel due to any personal issue, ONGC may recoverthecost/ expenditure
borne on behalf of the crew member from the Purchase Order / Contract issued to contractor against which the requirement
for manifest was given.
Crew personnel shall not engage in any kind of misconduct / illegal activity/ bad behaviour/ mischief with Hotel Staff/ ONGC
representative / Medical persons while staying at ONGC provided quarantine hotel and during crew change at Offshore. Any
violations by contractors and/or Service Providers will face consequences and contracts terms & conditions will be applied
subject to a maximum level to Suspension and /or Cancellation of the Contract/Agreement

d In case, any of our crew member is tested positive, M/s. OFFSHORE MARINETECH PVT. LTD shall make necessary
arrangements to evacuate him from the Hotel premises within 12 hours from receipt of COVID-19 positive Report.

In case, M/s. oFFSHORE MARINETECH PVT. LTD fails to evacuatethe crew member by 12.00 PM of next day from Covid-19
Report Generation date, penalty of Rs.10,000 per Day may be levied upon us and cost shall be recovered from the ongoing
Contract/ PO NOA No. MR/MH/MM/LSTK/SM/GI HRD LNE/2018-19/15 KC 19002/9030009674 D.30.01.2020.
