PURPOSE
The Company make The journey & Management Plan to design and to set a process that
we follow for planning and undertaking road transport journey in compliance with HSE
requirements with our goal which is Arriving Safely.
during RIG Move Mobilization & Risk Assessment from KRE-06 site to JAS-A2 site
location in West of Java.

SCOPE
The Journey Management Plan and procedures apply to all JO Harmand - Transporter (CV. MSU)
Subcontractors personal who are involved in RIG Move Mobilization. JO Harmand - CV. MSU Safety
Management Systems shall apply to all facets of the transport operation. JO Harmand - CV. MSU will
be responsible for providing work direction and will be the Primary Contractor for RIG Move
Mobilization activities.
RISK MANAGEMENT
A detailed RIG Move Mobilization & Risk Assessment was developed and a risk assessment review meeting was conducted prior to the development of this plan;
RIG Move Mobilization & Risk Assessment meeting involved key personal and representative from JO Harmand - CV. MSU the Transport Subcontractor. Risk
related to the prescribed road route have been assessed and documented. Subsequent hazards were identified, current controls were identified and proposed
controls have been detailed for each hazard. The risk assessment was developed from a road survey which was conducted on April 7, 2018.
For activities involving loading and unloading of the trucking JO Harmand - CV. MSU as Transport Contractors personnel shall take part in the development of
specific Job Safety Analysis (JSA) for all these activities. These JSA’s shall be documented and communicated so as to identify and raise awareness of the
hazards and the controls measures required to reduce the risk of injury to personnel and damage to equipment and the environment.
RESPONSIBILITIES OF KEY PERSONNEL
HSE Coordinator
The HSE Coordinator is the JO Harmand - CV. MSU representative onsite; he is responsible for the health and safety of all his employees as well as all of the
subcontractor’s personnel onsite. The HSE Coordinator is responsible for directing all RIG Move Mobilization& Risk Assessment related activities onsite JO
Harmand - CV. MSU Personnel. Shall be responsible for the scheduling of trucks and loads from KRE-B Indramayu site to JAS-A2 Subang well site location. The
HSE Coordinator shall make sure that the loading operation including RIG Move Mobilization and contractor’s equipment on to prime mover trailers or Low bed is
conducted in a safe and efficient manner. The HSE Coordinator shall be responsible for implementing the safe systems of work that are required such as Job
Safety Analysis (JSA), Permit to Work (including the implementation of Lifting and rigging procedure) and Pre-Job Safety Meetings for the loading and unloading
tasks. The HSE Coordinator is responsible for make sure that the hazard reduction measures are communicated to those persons doing the jobs.

Transport Subcontractor Owner


The transport subcontractor is responsible for the supply of trucks, low bed & Crane or other type of truck as adjusted with goods size. All equipment must be in
safe operating condition. All vehicles and equipment must meet with all statutory requirements for certifications of vehicles and equipment; such as registration
and certification. It is the subcontractor’s responsibility to provide competent personnel who hold the relevant certification to operate required equipment or carry
out any driving, crane operations, rigging or slinging tasks and all personnel must use complete PPE.

The HSE Officers /HSE Coordinator


Are responsible for conducting pre-job/loading safety meetings and for assisting in the development of the JSA’s and Permits to Work. The HSE Officer/HSE
Coordinator shall also conduct inspections of the lifting slings prior to use and shall also monitor and advise on Rigging and Slinging procedures for the loading
and unloading of trailers. The HSE officer/HSE Coordinator must always ready in both locations as the operation will work 24 hours.
Crane Operators
Crane is to be inspected prior to operation. The operator is responsible for conducting pre-start inspections on their equipment using JO Harmand - CV. MSU daily
inspection. Inspection shall include mechanical items and lifting tools/cables prior to any lifting activity. The operator is also responsible for operating Crane in
accordance with manufactures recommendations
The Truck Drivers
Truck drivers are responsible for conduct pre-start checks on the prime movers and trailers. Brakes, tires and suspension must be in safe working condition prior
to the loading operation. The truck driver shall be responsible for making sure that all loads are secure and only ratchet type load binders are used. The truck
driver is responsible for driving the vehicle in a safe manner at all time and shall obey all mandatory road signs such as speed limits. The driver shall not work
more than 14 hours per day including loading, driving and unloading in order to reduce the potential effects of fatigue. All drivers should have valid driving license
for the vehicle they are assigned to.
Wide loads
All loads that overhang the normal width of the low bed (L 12m x W 3,3m x 1m) & High Bed trailer (L 12m x W 2,3m x 1,5m) must be clearly identified. For
transporting wide loads during daylight hours red flags must be secured to the widest point on either side of the loads. When transporting wide loads during the
hours of darkness, the wide load must be identified by attaching red/yellow lights to either side of the loads widest points.

Checking of Load Heights


Prior to any truck departing the location the height of the load must be correctly checked, load heights are not to exceed 4 meters to load on the low bed and
exceed 2,8meters to load on the trailers. A loads heights test portal or gate must be assembled in the location for measuring all load heights on trailers before
they depart. The measuring portal or gate must be constructed so that it can accurately measure the required maximum height of the load. To avoid injury to
personnel or damage to equipment the measuring portal should be constructed from bamboo or similar lightweight materials (not steel). No person must be
allowed to be standing in close proximity to the fixture when trucks are passing through the portal in the event that the fixture or part of the fixture falls down due
to collision with the load.

Using Mode Transportation


Applies to all loads of the Generator Equipment Mobilization will use with as follow:
1. Low Bed with deck space is L 12m x W 3,3m x H 1m,
2. Trailer High Bed 40ft with deck space is L 12m x W 2,3m x 1.5m

Start from KRE-B Indramayu site to JAS-A2 Subang well site location, and also 2 units Crane 50 T capacities to loading or offloading process in yard/site location

LADDERS: USE AND SAFETY


Portable extension ladders have the following requirements:
1. Pitch at a slope of not less than an angle of 20 ̊
2. The ladders should extend at last 900 mm above the surface where the person can gain access
3. The ladder should be positioned on a level, firm and non-slip surface
4. If the person is working on the ladder, then he should be attached to top of the ladder and assistant should be standby at the base of the ladder to help
secure it.
PERSONAL PROTECTION EQUIPMENT (PPE)

 Hard Hats to protect from falling objects


 Boots and Shoes can provide a degree of
protection small cuts,bruises and abrasions.
 Gloves can protect hands from rough and
sharp parts.
 Safety Clothing as a fire retardant.
 Safety glasses provide eyes protection from
flying chips and debris.

COMMUNITY AWARENESS
General Freight and RIG Move Mobilization shall be carried out with the upmost, respect, safety, care and concern for the local communities that reside along the
different sections of the prescribed road route.
All due care and responsibility shall be discharged by JO Harmand - CV. MSU to make sure that the needs and concern of local communities are attended to and
that any matters or issues in relation to the General Freight and RIG Move Mobilization transportation operation are addressed by KRE-B Indramayu site to JAS-
A2 Subang and JO Harmand - CV. MSU
EQUIPMENT INSPECTIONS
Prior to equipment being use on the site location by the subcontractors, an inspection will be carried out; this inspection must be done prior to work being
undertaken and copies are to be kept on site.
The Subcontractors will be required to demonstrate that their vehicles and equipment are maintained to an acceptable safe standard by submitting the
appropriate inspection certificates or paper work to JO Harmand - CV. MSUfor each item that is intended for use. This inspection report is to becompleted by an
appropriately qualified person.

PROCEDURES FOR VEHICLE SAFETY – GENERAL


All trucks and Low Bed shall be operated in accordance with this procedure will be made inspection to save for General Freight and RIG Move Mobilization.
Before any vehicle is operated it is the responsibility of the driver to carry out a pre-start check on the vehicle for such items as tire condition, lights, brakes, horn,
wiper blades, indicator lights, seat belts
The road from KRE-B Indramayu to JAS-A2 Subang site passes towns and villages – drivers must drive carefully and restrict speed in order to prevent accidents,
reduce dust, and reduce noise
Drivers and other subcontractor personnel for transportation must attend safety briefing conduct before starting to General Freight and RIG Move Mobilization.

Journey management planning – primary ownership is with the individual driver. All JO Harmand – CV. MSU and subcontractor employees are expected to
behave as professional drivers when they are operating a vehicle.
Only personnel who hold a valid driver’s license shall be permitted to drive a truck and or operators Drivers shall operate vehicles in a safe manner and drive
to the condition of the road.
All vehicle drivers and authorized passenger shall wear safety belts and shall not drive the vehicle until all persons in the vehicle are wearing their seat belt
correctly.
Disciplinary measures shall be taken against any person found to be not using a seat belt whilst operating or traveling in a motor vehicle / truck. Disciplinary
measures shall also be taken against the driver of any vehicle who allows passengers to ride in the vehicle without the use of a seat belt.
Vehicles shall be safely parked with the parking brake engaged and the transmission in first gear; in the event that a vehicle needs to be parked on an incline
(gradient) one rear and one front wheels shall be sufficiently “chocked” with the use for vehicles Chocks or the use of an appropriate alternate.
Drivers shall only transport passengers who are associated with the operations.
DRIVERS & OPERATOR WORK HOURS
 No driver & Operators should work more than 12 hours per working period.
 No person employed in the capacity of driver should undertake any other major task as follows:
 Before, during or after their work period
 During their rest break(s)

SAFE DRIVING PROCEDURES


 Traffic Signs : All drivers must focus primarily on traffic
signs and signal.
 Seat belts : Seat belt must be used properly while
driving.
 Distracted Driving : when driving, drivers should
refrain from texting and talking on the phone unless
they are using a hands-free device.
 Speeding : When driving, drivers must follow the
speed limit.
 Drinking and driving : Driving while intoxicated is so
hazardous.
 AggresiveDriving : Drivers must stay away from other
aggressive drivers to avoid a crash or altercation.
LOADING AND UNLOADING DURING HOURS OF DARKNESS


Unloading during the hours of darkness will only commence once it has been agreed that sufficient lighting (light towers and portable lighting generator) have
been put in place.
Loads arriving during the hours of darkness at the JAS-A2 Subang Site location will not be unloaded unless having appropriate lightning and supervisor on site.
All low bed and trailer at night must have rotating lights, and all corners of the cargo properly marked with reflective tapes.
Ensure that all low bed, trailers and light vehicle lights are working Drivers are to park their trucks and loads in the designated
parking area.
All drivers must wear full PPE when they are on the JAS-A2 Subang site location (outside of their trucks, drivers and helpers must use high visibility at all time).
PERMIT TO WORK SYSTEM
The scope of the JO Harmand – CV. MSU permit to Work (“PTW”) system covers all JO Harmand–CV. MSU activity and operations. It is anticipated that all non-
routine activitiesinvolving potential risks will be subject to the same PTW system this includes heavy and or difficult lifts. Heavy lifts are defined as per the
description on the heavy and difficult lift permit
This procedure applies to all parties. In the event that JO Harmand – CV. MSU or subcontractor personnel requires a PTW to undertake activities of a
maintenance nature, they will advise the HSE Coordinator of the repair requirements, and will decide whether or not the equipment will need to be moved offsite
to conduct the work. If the equipment is not moved offsite it will be necessary to obtain a PTW before commencing the maintenance or repair work.
Note: A permit to work will be required for the use of all web slings.

The following permits are used under the JO Harmand – CV. MSU PTW system;
 General All Well Site Permits
 Cold Work Permit and confined space entry
 Hot Work Permit and confine space entry
 Preparation/Isolation Permit
Lift Permit need to be developed to reduce the hazards before conducting difficult or heavy lifts, difficult lifts can mean the use of two cranes in tandem to lift
an item of the General Freight and RIG Move Mobilization. The HSE Coordinator will have the authority to define difficult lift situations.
DRUGS & ALCOHOL
It is strictly forbidden to drive a vehicle under the influence of alcohol, narcotic and psychotropic substance and / or medicines that may affect the driver’s
capacity to safely operate the vehicle.
JO Harmand – CV. MSU has a zero tolerance (0,0%) for alcohol affected personnel involved in driving activities, and disciplinary measures will be taken
against any person who is found to have driven a motor vehicle and or truck whilst under such influence from the consumption of alcohol.
JO Harmand – CV. MSU has a zero Drug abuse tolerance for the use of by any person employed or subcontracted and or involved with JO Harmand – CV.
MSU operation including General Freight and RIG Move Mobilization.
Pre-Mobilization Checklist:

Personnel:
1. Copy of equipment operators’ current drivers’ license.
2. Any documentation required to permit the use of the equipment for the mobilization.
3. Any documentation required to permit the transport of the equipment.
4. Personnel Protective Equipment (PPE). Safety hard hat, glasses, shoes, gloves and ear plugs for all personnel.
5. Food and water for all personnel.

Trucks & Trailers:


1. Third party truck road worthy inspections
2. Copy of current registration.
3. All truck tires in good condition
4. Spare tire & tire changing tools – jack, levers, and wheel sockets
5. Truck and trailer brakes; fully functional
6. Headlights; two, able to be dipped
7. Tail lights
8. Stop lights
9. Turn signals; front and rear.
10. Windscreen wipers and washers
11. Mechanically sound in a road worthy condition, free of structural defects.
12. Breakdown Triangle kits – reflectors
13. First Aid kit
14. Fire extinguishers

16. Escort driver must have hand-phone – not to be used when escort vehicle is moving.
17. Truck drivers must have hand-phone – not to be used when truck is moving.
18. Trailers are to be fully decked as intended.
19. Sufficient loads restraints to secure the variety of loads that will be moving. (Minimum Requirement is four sets of chains plus tensioning devices sufficient
wood to pack and bolster loads).
20. Transport subcontractor to make vehicles & equipment available for inspection prior to acceptance.
21. All fuel and oil required to keep equipment operational.
22. Full insurance

LOADING AND UNLOADING OPERATIONS Prior to Operation


1. JO Harmand–CV. MSU to conduct an audit on transport Subcontractor prior to General Freight and RIG Move Mobilization.
2. Trucking contractor shall be aware of and obtain all government regulations and permits required (about height and width limitation)
3. The subcontractor shall be responsible for all damage incurred as a result of driver and or supervisor negligence.
4. The subcontractor shall be responsible for all damage to JO Harmand–CV. MSU and Third Party equipment between locations if as a result of failing to
follow instruction and or negligent.
5. All incidents must be reported using the JO Harmand–CV. MSU First
6. Operations are based on a (24) hour working day by shift rotation
Sufficient flood lighting must be in place prior to any activity after dark that involves loading at Cipondoh Yard and unloading at KRE-B IndramayuI site
location.

JAS-A2 Subang Well Site Location sopar its ok and good condition of road and extra caution required
when passing fuel station, Poor condition roads, narrow road, and road full of traffic,markets,
school and frame of bridge.

Scope
A risk assessment covers the loading at Petapahan Kampar Raiu, along the route from KRE-B
Indramayu to JAS-A2 Subang well site unloading at Well site, and supporting equipment required for
handling General Freight and RIG Move Mobilization.

Purpose
The purpose of these risks assessment is to describe the steps to be taken to ensure the equipment
being mobilize from KRE-B Indramayu to JAS-A2 Subang well site Location is properly controlled and
carried out in a safe manner at all times.
